At Elite Physical Therapy we are happy to work with insurance to cover the cost of your care. We are a preferred provider for many regional and nationwide companies.
After you provide us with your insurance information, we will contact them to learn the specific benefits of your plan, what services have been used this year, and if there are any limitations on therapy services. If your carrier requires a co-pay, deductible, or co-insurance payment, we will notify you of that at the time of your first visit.
We will also verify whether your carrier requires a referral. If required, we ask that you provide your referral at your first appointment to avoid any billing problems.
If you DO NOT have insurance or a third party is responsible for payment of your injury, please let us know when you schedule your first visit so we can discuss payment options prior to your evaluation.
