At Boost PT & Sports Performance, you have options for your care. When you fill out the appointment request form below, you may choose from three types of appointments: An in-person evaluation at our clinic, virtual appointment, or free screening.
Access full care quickly through our online form. A clinic coordinator will be in touch with you shortly to set up your initial appointment. These appointments are typically between 60-90 minutes and require some paperwork to be completed prior to being seen by a physical therapist.
