At Accelerated, you have options for your care. When you fill out the appointment request form below, you may choose from three types of appointments: An in-person evaluation at our clinic, virtual appointment, or free screening.
Access full care quickly through our online form. A clinic coordinator will be in touch with you shortly to set up your initial appointment. These appointments are typically between 60-90 minutes and require some paperwork to be completed prior to being seen by a physical therapist.
Our clinic(s) offer free screening appointments for those who may not be ready or financially able to commit to a physical therapy treatment plan. Our screenings take about 15 minutes and help people understand the role physical therapy may or may not be able to play with their concerns or condition.
